Injuries & Diseases
Life on the frontier leaves marks. Gunfights leave you bleeding. Swamp camps breed malaria. Cold rivers bring fevers. A snake's fangs can kill in minutes. This guide covers every condition, how it escalates, and exactly what you need to treat it โ with or without a Doctor.
Overview
Rosewood Ridge uses a detailed medical system that tracks real diseases and physical injuries. These conditions don't just drain your health โ they impose escalating debuffs as they worsen. Untreated bleeding can kill in minutes. Wound infection causes fever and ragdolling. Disease zones near swamps can give you malaria if you linger too long.
All conditions have three severity stages. Most progress automatically if left untreated. Some โ like cholera and malaria โ never heal on their own and require a Doctor's intervention.
Severity Stages
| Stage | Colour | Description |
|---|---|---|
| 1 โ Mild | โ Green | Noticeable but manageable โ time to treat |
| 2 โ Moderate | โ Amber | Significant impairment, health drain worsens |
| 3 โ Severe | โ Red | Potentially lethal โ does not progress further but does not heal naturally |
All injury and disease conditions are wiped completely when you respawn. If no Doctor is available and your condition is unmanageable, respawning is a valid last resort โ but you lose the RP continuity of your current scene.
Bleeding
Caused by: Gunshots, knife/melee wounds, arrows, and most weapon impacts against your character.
Effects by Severity
| Stage | HP Drain/Tick | Tick Rate | Escalates After |
|---|---|---|---|
| 1 โ Mild | 10 HP | Every 5 sec | ~2 min |
| 2 โ Moderate | 25 HP | Every 5 sec | ~4 min |
| 3 โ Severe | 50 HP | Every 5 sec | Stays until treated |
Bleeding auto-heals after ~5 minutes at any stage if untreated, but in the meantime the constant health drain at Severe will put you down. A bandage is the fastest fix โ always carry one.
Treatment
| Item | Notes | Duration | Where to Get |
|---|---|---|---|
| Bandage | Standard โ stops the bleed, may reopen after 60s | 60s temp / Full cure | General Store ยท Doctor |
| Herbal Bandage | Herbal treatment, slightly longer lasting | 90s temp / Full cure | General Store ยท Doctor |
| Army Bandage | Strongest bandage โ permanent cure | Permanent cure | Doctor Clinic |
| Tourniquet | Emergency stop โ cuts off blood flow entirely. Requires Doctor removal. | Permanent stop | Doctor Clinic |
A tourniquet stops bleeding immediately but leaves residual effects until a Doctor removes it properly. It's an emergency measure โ always follow up with a Doctor when safe to do so.
Wound Infection
Caused by: Untreated wounds over time. If a wound isn't properly cleaned and dressed, infection sets in. Does not auto-heal โ requires treatment.
Effects by Severity
| Stage | HP Drain/Tick | Stamina Effect | Extra Debuffs |
|---|---|---|---|
| 1 โ Mild | 5 HP / 1s | Stamina depletion ร1.5 | None |
| 2 โ Moderate | 15 HP / 1s | Depletion ร2, Regen ร0.5 | 10% chance of fever ragdoll when sprinting |
| 3 โ Severe | 30 HP / 1s | Depletion ร3, Regen ร0.3 | Sepsis โ high ragdoll chance (15%) when sprinting |
Wound infection is one of the most dangerous conditions. It does not auto-heal. At Severe (sepsis) you will struggle to sprint without going down.
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antibiotic | 80% chance of cure | Doctor Clinic |
| Herbal Medicine | 30% chance of cure | General Store ยท Doctor |
| Morphine | 100% โ Doctor administers | Doctor only |
| Health Elixir | 60% โ Doctor administers | Doctor only |
Broken Bone
Caused by: Heavy fall damage, bullet impacts to limbs at high force (70+ damage), explosions. Does not auto-heal.
Effects by Severity
| Stage | Running Damage | Escalates After |
|---|---|---|
| 1 โ Mild | 50 HP when sprinting | ~5 min |
| 2 โ Moderate | 100 HP when sprinting | ~10 min |
| 3 โ Severe | 150 HP when sprinting | No further progression |
Running on a broken bone causes direct direct HP damage proportional to the stage. At Severe, sprinting rapidly depletes health โ you effectively cannot sprint without dying.
Treatment
| Item | Effect | Where to Get |
|---|---|---|
| Splint | Heals 40% of bone injury โ reduces running damage significantly | General Store ยท Doctor |
| Doctor Treatment | Full clear of the broken bone | On-duty Doctor |
Common Cold
Caused by: Spending time in cold temperatures, staying wet (in rivers/rain) without warming up. Coldness builds up over time โ once you reach the threshold, the cold sets in. Auto-heals after ~30 minutes.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 10 HP | ~5 min | Occasional coughing |
| 2 โ Moderate | 30 HP | ~10 min | More frequent coughs, elevated heart rate |
| 3 โ Severe | 50 HP | Auto-heals ~30 min | Elevated fever (37.8โ38.5ยฐC), heavy coughing |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antibiotic | 100% | Doctor Clinic |
| Herbal Medicine | 50% | General Store ยท Doctor |
| Herbal Tonic | 30% | General Store ยท Doctor |
| Special Health Tonic | 50% | Doctor Clinic |
| Morphine | 30% โ Doctor administers | Doctor only |
Stay dry and warm. Avoid swimming in cold rivers for extended periods without warming up afterwards. Wearing layered clothes adds body temperature, reducing how fast the cold accumulates.
Snake Bite
Caused by: Being bitten by a snake. Does not auto-heal โ the venom must be neutralised with an antidote.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 15 HP (~1.25s tick) | ~2 min | Minor nausea, elevated heart rate |
| 2 โ Moderate | 25 HP (~1.25s tick) | ~4 min | Vomiting animation, heart rate extreme |
| 3 โ Severe | 40 HP (~1.25s tick) | Doesn't auto-heal | High fever (40โ41ยฐC), extreme distress |
At Severe, you lose ~32 HP per second. Without an antidote you will go down very quickly.
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antidote (Anti-Poison Remedy) | 100% | Doctor Clinic |
Snake encounters in the wilderness are sudden. By the time you've reached a Doctor at Severe stage, you may already be down. Carry an Antidote from a doctor clinic whenever venturing into grasslands, swamps, or forests.
Malaria
Caused by: Spending time in malaria-infected zones during an active Malaria outbreak โ primarily the Roanoke Valley swamp areas. Infection builds up while in the zone. Does not auto-heal. Outbreak-only: infection cannot build up between outbreak events.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 20 HP/s | ~5 min | Weakness, nausea |
| 2 โ Moderate | 40 HP/s | ~10 min | Vomiting, high fever (38.5โ40ยฐC) |
| 3 โ Severe | 60 HP/s | Doesn't auto-heal | Extreme fever, persistent vomiting |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Insect Bite Remedy | 80% | Doctor Clinic |
| Morphine | 50% โ Doctor administers | Doctor only |
| Health Elixir | 60% โ Doctor administers | Doctor only |
Avoid spending long stretches in the Bayou Nwa or Lagras areas without a remedy on hand. Even a short visit won't infect you โ but prolonged activity (hunting, fishing in the swamp) will. Keep an Insect Bite Remedy in your kit if your char spends time in those areas.
Cholera
Caused by: A contaminated zone in the far west โ active during a Cholera outbreak event only. Infection spreads very rapidly (low threshold). Does not auto-heal. Transmissible to nearby players. NPC contagion also adds risk even outside the zone.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 40 HP/s | ~5 min | Severe stomach pain, nausea |
| 2 โ Moderate | 60 HP/s | ~10 min | Vomiting, very high fever (39โ41ยฐC) |
| 3 โ Severe | 80 HP/s | Doesn't auto-heal | Extreme distress โ lethal without treatment |
Cholera is the fastest-escalating disease. The infection zone has a very low exposure threshold โ even brief exposure can trigger it. Get to a Doctor immediately.
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antibiotic | High | Doctor Clinic |
| Herbal Medicine | Moderate | General Store ยท Doctor |
| Health Elixir | 60% โ Doctor administers | Doctor only |
Dysentery
Caused by: Environmental contamination and NPC proximity โ active during a Dysentery outbreak event only. Slow but persistent. Auto-heals after ~1 hour if untreated. Wearing a bandana does not help (fecal-oral spread).
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 2 HP/s | ~5 min | Stomach cramps |
| 2 โ Moderate | 4 HP/s | ~10 min | Vomiting, elevated heart rate |
| 3 โ Severe | 8 HP/s | Auto-heals ~1h | Persistent vomiting, mild fever |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Herbal Medicine | 80% | General Store ยท Doctor |
| Antibiotic | 50% | Doctor Clinic |
| Health Elixir | 60% โ Doctor administers | Doctor only |
Tuberculosis (Outbreak Only)
Caused by: Prolonged close contact with infected NPCs in Valentine or Saint Denis during an active TB outbreak. Very hard to contract โ requires sustained exposure (~15โ30 minutes in zone without a bandana). Wearing a bandana significantly reduces risk.
Tuberculosis is the only disease on Rosewood Ridge that does not clear when you respawn. Once contracted, it remains until cured โ or until your character's body fights it off (which can take ~4 hours and is not guaranteed). There is no reliable cure. See a Doctor as soon as possible.
Effects
| Symptom | Frequency | Notes |
|---|---|---|
| HP drain | 5 HP every 3 minutes | Very slow, chronic โ won't kill quickly but accumulates |
| Coughing fits | Every 3 minutes | Persistent respiratory animation |
| Dizzy/collapse episode | ~20% chance per 3-min tick | Brief stumble/ragdoll โ simulates severe fatigue |
| Stamina debuff | Constant while active | Depletion ร2, regen ร0.5 โ chronic fatigue |
| Body temperature | Constant | Low-grade fever 37.8โ39ยฐC |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antibiotic | 5% per dose โ very unlikely | Doctor Clinic |
| Natural recovery | Possible after ~4 hours of illness | No item โ rare luck-based survival |
Typhoid Fever (Outbreak Only)
Caused by: Spending time near contaminated water sources in Saint Denis docks or Thieves' Landing during an active Typhoid outbreak. NPC proximity also increases exposure. Wearing a bandana reduces risk. Does not auto-heal.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 55 HP/s | ~5 min | Stomach pain, nausea, elevated heart rate |
| 2 โ Moderate | 75 HP/s | ~10 min | Vomiting, stomach cramp animation, high fever (39.5โ41.5ยฐC) |
| 3 โ Severe | 95 HP/s | Doesn't auto-heal | Extreme distress โ requires urgent Doctor attention |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antibiotic | 50% | Doctor Clinic |
| Herbal Medicine | 20% | General Store ยท Doctor |
| Morphine | 40% โ Doctor administers | Doctor only |
| Health Elixir | 90% โ Doctor administers | Doctor only |
Yellow Fever (Outbreak Only)
Caused by: Spending time in mosquito-infested swamp zones โ southern Bayou Nwa and Flat Iron Lake south shore โ during an active Yellow Fever outbreak. Mosquito-borne: a bandana provides no protection. Does not auto-heal.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 60 HP/s | ~5 min | Extreme fatigue, nausea, rapid heart rate |
| 2 โ Moderate | 80 HP/s | ~10 min | Vomiting, very high fever (40โ41.5ยฐC) |
| 3 โ Severe | 100 HP/s | Doesn't auto-heal | Extreme fever โ rapidly fatal without treatment |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Insect Bite Remedy | 50% | Doctor Clinic |
| Antibiotic | 40% | Doctor Clinic |
| Herbal Medicine | 20% | General Store ยท Doctor |
| Health Elixir | 80% โ Doctor administers | Doctor only |
Yellow fever is transmitted by mosquitoes, not through respiratory contact. Wearing a bandana will not reduce your exposure. Avoid the swamp zones entirely during an active Yellow Fever outbreak if you don't have supplies.
Influenza (Outbreak Only)
Caused by: Close proximity to infected NPCs anywhere on the map during an active Influenza outbreak โ no fixed zone. Spreads through respiratory droplets. Wearing a bandana blocks transmission. Auto-heals after approximately 90 minutes.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 30 HP/s | ~5 min | Fatigue, coughing fits, fever 38โ39.5ยฐC |
| 2 โ Moderate | 50 HP/s | ~10 min | Persistent cough, fast heart rate |
| 3 โ Severe | 70 HP/s | Auto-heals ~90 min | Severe fatigue โ can survive without treatment |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Antibiotic | 80% | Doctor Clinic |
| Herbal Medicine | 60% | General Store ยท Doctor |
| Herbal Tonic | 30% | General Store ยท Doctor |
| Medicine | 30% | General Store |
| Health Elixir | 70% โ Doctor administers | Doctor only |
| Rest (no treatment) | Auto-heals ~90 min | โ |
Influenza is the only major outbreak disease where a bandana directly blocks transmission. Putting on a face covering before visiting populated areas during an active flu outbreak will prevent exposure entirely.
Heat Stroke
Caused by: Being in hot temperatures, in direct sunlight (sunlight multiplies the effect 1.5ร), and sprinting/exerting yourself in the heat (exertion adds 2ร to heat buildup). Auto-heals after ~20 minutes if you cool down.
Effects by Severity
| Stage | HP Drain/Tick | Escalates After | Symptoms |
|---|---|---|---|
| 1 โ Mild | 15 HP (~0.4s tick) | ~4 min | Dizziness animation, fever 39.5ยฐC+ |
| 2 โ Moderate | 30 HP (~0.3s tick) | ~8 min | Severe dizziness, blurred vision |
| 3 โ Severe | 50 HP (~0.25s tick) | Auto-heals ~20 min | Extreme heat fever (up to 42ยฐC), collapse risk |
Treatment
| Item | Cure Probability | Where to Get |
|---|---|---|
| Restorative | 50% | General Store ยท Doctor |
| Potent Restorative | 100% | Doctor Clinic |
| Stimulant Tonic | 30% | Doctor Clinic |
| Herbal Tonic | 30% | General Store ยท Doctor |
| Special Health Tonic | 50% | Doctor Clinic |
Stay in the shade during hot weather. Avoid sprinting under direct sunlight in New Austin or other hot regions during the peak heat hours. Carry a Restorative if you're planning long rides through desert terrain.
Reviving Downed Players
When a player is unconscious and unable to get up, a Doctor can revive them in the field using one of two items:
| Item | Who Can Use | Apply Time | Notes |
|---|---|---|---|
| Syringe | Doctor only | ~20 seconds | Primary revive โ full health restoration, complex animation |
| Lavender Smelling Salts | Doctor only | ~15 seconds | Secondary revive โ faster, slightly less health restored |
If no Doctor is available, you can attempt a self-revive by typing /sendhelp when downed.
Type /calldoctor when downed to send out a general alert to all on-duty Doctors. Your position will appear on their map. If no Doctor answers and your situation is dire, use /sendhelp for the self-revive NPC.
